I've gotten a couple emails asking "where does incident response fit in?" I believe that's a separate process that comes before Root Cause Analysis. I would call it Incident Management, and it would also be composed of three phases -- Response, Mitigation, and Assessment. That last phase is the key that determines whether Root Cause Analysis is performed. This may be the subject of a future article.
