| Hi Colin, I am not aware of any off-the-shelf software packages that allow you to "... input incident statistics and details to get a project overview of trends and the why's of incidents and how we can stop repeating these." Every system I've seen was basically a custom-built database with lots of manual input. Even then, the trends do not come completely automatically... somebody has to slog through and apply some intelligence. I've done it, and its not fun. I have heard of some datamining tools that may do some of what you want. I believe it would still require some manual input, but it might take some of the grunt work out of finding trends.
