| This is just an incredible series of articles. Root Cause Analysis of Human Error and Tangible Defects just happens to be what I am getting into. Being a PhD Chemist gives me a good dose of intuition plus a unique way to look at problems and understand graphs and diagrams, but I just did not have a systematic way to approach complex problems like the ones I have been hired to solve. | I have not tried your method yet, but totally it is a great way of starting investigate accident for commond men. My idea, accident investigation should not be framed with historic theory. |
